When should I see a nephrologist or kidney specialist?

You may need a nephrologist if you have reduced kidney function, protein or blood in the urine, difficult-to-control high blood pressure, electrolyte problems, recurrent kidney issues, or chronic kidney disease. Referral may also be recommended based on abnormal blood or urine tests.

Kidney disease can have no symptoms early on. Possible signs include swelling in the legs or around the eyes, changes in urination, fatigue, nausea, itching, poor appetite, or persistent high blood pressure, but blood and urine tests are often needed to detect early disease.
Diabetes can damage the kidney’s filtering units over time, while high blood pressure can damage kidney blood vessels. Controlling blood sugar and blood pressure can help reduce the risk of kidney damage and slow progression.
Kidney function is commonly evaluated with blood tests such as creatinine and estimated glomerular filtration rate (eGFR), urine tests for protein or albumin, and sometimes imaging such as ultrasound. Additional tests may be ordered depending on the suspected cause.

Dialysis may be needed when kidney function becomes severely reduced and the kidneys can no longer adequately remove waste, manage fluid, or balance electrolytes. The decision is based on symptoms, laboratory findings, overall health, and kidney function rather than a single test result.
